No I can’t, you can come to me or send someone to my house I’m physically in alot of pain and I’m not trying to travel anywhere right now I can barely walk I’m regressing on all the milestones I’ve accomplished and am trying to focus on staying alive.

There is no common area there is the house or outside I'd prefer if you came in as the pca lady did, I thought as a mandated reporter you were supposed to see the condition of my apartment anyway. When you say these things it makes me feel like less of a human being because i never did anything but tell you the truth, if u feel unsafe then all the hours you are billing my medical assistance you can take a friend, but ive never threatened you, even the pca lady came at 8pm and sat on my carpet with me, you my cady waiver case manager who ive never met once wants to not come in my place of residence but had no problems okaying 1000 units for it to be clean so maybe before the day you want to come do whatever paperwork you can have the cleaning company do the 999 hours that they haven't done yet since the end of june when u said they had been approved. Then maybe the conditions I live in will be more up to your standards to come see me and treat me like a real person. Any of those days work please let me know a specific time and day so that I can know to prepare myself. I have papers from ramsey county that I believe go along with that whatever we are doing I don't understand why they are asking for my credit information so I want to make sure that I fill them out with someone from the county they said they are for minsure. Also i have not received the 45 dollar check for the application fee I want to make sure it's going to my mailing address as we spoke several times about my mailbox at the apartment continues to be broken into as alot of other boxes are so any mail or packages you are sending I'll ask again please send them to my mailing address.

Hello Elana and Folk at People Incorporated:
Thank you for keeping me looped in.
Video meetings can be held, they are allowable. If that is possible for now, I highly recommend that option. At least Elana and Jessica will be able to see each the other and interact that way. I think it will be important to discuss how to best communicate and plan with Elana. It seems there are services authorized but I hear Elana expressing needs that aren’t being met, meaning coordinating with providers to accomplish service delivery.
Elana has expressed to me good solutions and tips in working with and communicating with her. She has expressed she’d like her ideas to be part of her support plan.
I am still working on scheduling a meeting with Ramsey County, People Incorporated, Elana, me and another metro Regional Resource Specialist, Kyle, (my team member) to discuss how to best support her. Some of the issues seem to be around how to best communicate. Has the team discussed with Elana authorizing Person-Centered Planning (under the Family Training and Counseling waiver service)? This is a trained planner to work with Elana, explore support options to help her meet her goals. Here is information on that option:
Person-centered planning facilitation
The lead agency may use this service to hire a facilitator to do the following activities during the person-centered planning process:
· • Understand who the person is
· • Establish and support a personal vision created by the person
· • Identify community supports and action items with the person to achieve their vision.

I will be in touch with you all next week to discuss connecting about where to go from here.

I had to go to the hospital for my legs over the weekend my nerve damage and arthritis was too much for me. I said in the previous email that whatever time worked for me for u to come to my house I believe you said the 1st 2nd 3rd or 4th of August I never heard about anything else. I saw some long message that whoever the jody person is initially sent then somehow disappeared from the thread, then some message yesterday saying she is willing to work with me but never saw a Time. Could we all call eachother today to figure this out because it makes it really difficult when I'm trying to get in to a routine and my health and stuff gets in the way but now I'm worried about whatever little things I haven't been receiving but yall already billed for is already approved and paid out but none of my concerns or case management issues are being addressed let alone resolved and then everyone at your company seems to put it on me instead of being accountable for your own consistently questionable and unclear actions.

I want to make sure we are clear, I am requesting that I am able to speak to everyone (my lady who is helping me from the state of Minnesota, my current case manager for my cadi waiver and whoever else is supposed to be involved in my case from a supervisor position) via telephone like a conference call to schedule a video call to get whatever paperwork is being requested for me to complete, and to ensure that what transpires from the communication is put into practice immediately, and the checks and balances are all being accounted for in writing and through these video or in person interactions so that we all can be the most effective. If we can't do that since this will be the 3rd strike with people's incorporated then I would choose to take my cadi waiver services to another provider that can best assist me, so then in that case I don't want my cadi waiver to be interrupted because the August review thing didn't get filed through the office or some reason that has been my constant concern and complaint throughout this entire 3yr process with state/county as a legally disabled person going through this web of inconsistent consistency of back and forth mixed signals miscommunication misunderstanding shift of blame and responsibility complacent inappropriate unhelpful damaging and destruction of trust, of integrity or credibility that is so counterproductive for not only the disabiled community but the whole state suffers so I understand how that seems so out of everyone's pay grade to actually just listen and comprehend the truth and the amount of innovation and positive impact my presence is demanding of anyone who encounters me In life, I know what I need to best assist my current condition and state of being but if the people who get paid to help me don't actually care about what is helpful to me as an individual, but also doesn't want the responsibility of having to redesign the structure of the why and for who, in alignment with the unified definition clearly outlined expectations and what will be given and received will be supportive and beneficial now, and achieve actual lasting self sustainability that is empowering to all the people impacted. If George Floyd didn't give his life to be the undeniable reason Minneapolis police misconduct could no longer hide behind the masquerade, finally all the claims of misconduct, abuse and ineffectivness of habitual behavior ignored actually investigated and paying out over 21 million dollars. The fact that their actions or inability to act violated the constitutional rights of "criminals " was the responsibility of the department they could have prevented so much. I'm not asking for much, i have a consistent and clear log of just how ineffective, deceptive and unjust ive been treated, and I believe that the entire disabled population in our state, especially dealing with Hennepin and ramsey county disability community services providing assistance for profit have neglected and violated all our human rights. I'm not just one person, and I'm saying these things because I care about myself and about the world and humanity and all life, unconditionally, but its so hard to live when everyone is living off of you. Canceling my cadi waiver services won't help the world, but please believe this is the last opportunity to work with me to create a better network that actually serves the entire state. I will be the George Floyd of this groundbreaking systematic injustice, I have nothing, so I have nothing to loose, but the gain will be the change I wished to see. If you all maybe can get together and decide a time that everyone has available, and then can notify me of the time the call will be scheduled, giving me a 24hr heads up and a clear time day and confirmation that we will all be attending this call, since I know how my brain injury in combination with my life right now is the best time of day is always between 10 and 12pm for important communication, so what I need is a actual time I can set a reminder and alarm and prepare for this ahead of time mentally physically and emotionally so I can trust that if you guys say ok 11am Thursday the 4th we will all be on the line and conference call you then I know what to expect and how to proceed on my end. I need that clarity and direction to literally get through brushing my teeth without forgetting what I'm doing or getting mentally over stimulated and my brain automatically shutting down its constant so please help me help you enhance my quality of life by communicating and connecting with me in the way that is most efficient and productive for my disability and my life circumstances.
